CCDPH Immunization Records

The Cook County Department of Public Health only has immunization records for people who received immunizations with our agency.

If you did not receive a vaccination with CCDPH and are looking for a copy of your immunization records, the Illinois Department of Public Health (IDPH) launched vax verify for residents to obtain their records.

To request CCDPH immunization records, please download, complete and return one of the following forms:
