The Center for Faith and Community Health Transformation

The Center for Faith and Community Health Transformation (The Center) was formed to transform the health of our communities through faith-rooted, collaborative action. We work to advance health equity by building community, nurturing leaders and connecting with the unique spirit power of faith communities to promote social justice and abundant life for all. The Center is a joint initiative of Advocate Health Care and the Office for Community Engagement and Neighborhood Health Partnerships at the University of Illinois at Chicago.

So living in a healthy community means that it has been built in a way that can improve your health. It can, for example, support you in being physically active; in buying affordable fresh fruits and vegetables; in breathing clean air; and in providing you with opportunities to connect with jobs, education, resources and other people in your community.

Our Involvement With Healthy Hotspot

The Center is partnering with and equipping faith communities in suburban Cook County to be leaders in shaping the culture of their communities to be healthy and just. We are highlighting best practices, offering support and technical assistance, and connecting people of faith with opportunities to lead for health.
