SEEKS SOLUTIONS TO AMERICA’S HEALTH CRISIS
The Cook County Department of Public Health is pleased to present 2005-10, documenting a broad health assessment and planning process conducted in partnership with suburban Cook County community providers and residents. The document is rich in useful public health data for the suburban Cook County communities including:
Community Health Report Card 2005—a regional data assessment of SCC examining 55 indicators and identifying trends and comparisons with state, US and Healthy People 2010 indicators.
Community Health Status Assessment—an extensive and descriptive examination of disease morbidity and mortality along with demographic, socioeconomic and behavior factors
Local Public Health System Assessment—a documentation of the current status of the public health system in SCC
Community Provider and Resident Surveys 2005—a compilation of both resident and community providers concerns and needs
Extensive appendices including data tables and demographic and mortality SCC maps.
Development of a Community Health Plan is documented in the second section of culminating in the prioritization of three health indicators:
Chronic Disease
Violence
Access to Health Care
The plan serves as a population-based, prevention-focused health improvement plan for the next five years.
