Targeted Intensive Prenatal Case Management

The Targeted Intensive Prenatal Case Management Program (TIPCM) focuses on the prevention of low birthweight infants to women who are considered at high risk for pre-term (premature) births. Special characteristics of the TIPCM include monthly incentives such as stroller and car seats, two client contacts per month of pregnancy and a community outreach plan to identify and recruit high risk women into the program.

Nurse Case Managers assist the client with:
• Accessing early entry into prenatal care.
• Assessment to identify medical needs, social concerns and barriers to care.
• Development of an individualized care plan to address client needs/concerns.
• Referral to appropriate medical and psychosocial services including WIC and Medicaid Presumptive Eligibility (MPE)/KidCare services.

Eligibility Criteria:
• Resident of Illinois who lives in Bellwood, Harvey, or Maywood.
• Established risk factors.
