The Targeted Intensive Prenatal Case Management Program (TIPCM)
focuses on the prevention of low birthweight infants to women who
are considered at high risk for pre-term (premature) births. Special
characteristics of the TIPCM include monthly incentives such as
stroller and car seats, two client contacts per month of pregnancy
and a community outreach plan to identify and recruit high risk
women into the program.
Nurse Case Managers assist the client with:
Accessing early entry into prenatal care.
Assessment to identify medical needs, social concerns and
barriers to care.
Development of an individualized
care plan to address client needs/concerns.
Referral to appropriate medical and psychosocial services
including WIC and Medicaid Presumptive Eligibility (MPE)/KidCare services.
Eligibility Criteria:
Resident of Illinois who lives in Bellwood, Harvey, or Maywood.
Established risk factors.
