The HealthWorks program provides Medical Case Management services
to children 5 years old and under and pregnant and parenting teens
who are in the care and custody of the Illinois
Department of Children and Family Services.
Program Goals: Establish a health history for the ward
Increase immunization compliance and regular well child exams
Ensure adequate and appropriate access to primary care and
medical specialty as needed.
These goals are accomplished through a medical case management
model that includes:
Coordination of comprehensive health care and services through
telephone communication with the Substitute Caregiver (SCG), the
Primary Care Provider (PCP) and the case worker.
Completion of an assessment, an individualized care plan.
Implementation of the care plan through referral and education
Monitoring and evaluating the process on a regular basis.
