HealthWorks

The HealthWorks program provides Medical Case Management services to children 5 years old and under and pregnant and parenting teens who are in the care and custody of the Illinois Department of Children and Family Services.

Program Goals:
• Establish a health history for the ward
• Increase immunization compliance and regular well child exams
• Ensure adequate and appropriate access to primary care and medical specialty as needed.

These goals are accomplished through a medical case management model that includes:

• Coordination of comprehensive health care and services through telephone communication with the Substitute Caregiver (SCG), the Primary Care Provider (PCP) and the case worker.
• Completion of an assessment, an individualized care plan.
• Implementation of the care plan through referral and education
• Monitoring and evaluating the process on a regular basis.
