The Family Case Management (FCM) program works to provide services
to pregnant women and infants that are below 200 percent of the
federal
poverty level through outreach, comprehensive case management
and support services to its clients.
Family case managers provide these services to the clients at clinic
sites and during home visits. Many of the infants that are eligible
for the FCM program are also eligible for the high-risk infant program,
High Risk Infant Follow-Up (formerly known as Adverse Pregancy Outcome Reporting System-APORS).
Goals: Assist clients gain access
to early prenatal and continue prenatal care throughout pregnancy
Increase immunization compliance and regular well child visits
Provide counseling and anticipatory guidance, referrals and
follow-up to needed services.
Provide nutritional assessment and counseling
Conduct periodic health assessment
These goals are accomplished through a case management model
that includes: Assessment to identify medical and
social needs
Identification of barriers to services
Development of an individualized care plan
Implementation of the care plan through education and referrals
Re-evaluation of client needs on a regular basis
