Cook County Connects is managed by the MidAmerica Center for Public Health Practice and Communities Putting Prevention to Work (CPPW). CPPW is made possible through funding from a grant administered by the Centers for Disease Control and Prevention, and is a joint initiative of the Cook County Department of Public Health and the Public Health Institute ofMetropolitan Chicago.
