Suburban Cook County School Vaccination Campaign

Influenza-like illness is widespread, and the 2009 H1N1 (swine) flu appears to be the predominant strain circulating. While this new H1N1 influenza virus has caused mild to severe illness resulting in hospitalizations and deaths in both children and adults, children have been most affected and are considered at high risk of infection. Vaccination is the best way to protect children from this potentially serious illness.

Cook County Department of Public Health (CCDPH) is working closely with schools and daycare centers throughout suburban Cook County to plan for the agency’s H1N1 vaccination campaign.

For Public and Private Schools:
CCDPH officials shared information with schools and school district administrators in September about the agency's upcoming school-based vaccination campaign.

To participate as a vaccination center, a signed Memorandum of Agreement is required.  For public school, this MOA should be signed by your School District Superintendent. For private (non-public) school, the MOA should be signed by your principal.

Please download and review the appropriate MOA below.

A signed copy of the MOA should be sent via mail to:

You may also fax a copy at (708) 492-2660 OR (708) 492-2133.
