Thank you to the more than 50 participants who have attended the Community Health Partner Committee meetings this Fall. It is through the community assessments and participant discussions that we learn more about both the strengths and gaps in the public health system’s abilities to improve conditions that support health for everyone.
All are invited to the last Community Assessment meeting scheduled for Thursday, January 21 from 11:00am – 3:30pm. Lunch will be provided. (Click on the attached .ics file in Documents tab to add the event to your Outlook calendar.) The agenda will include:
- a presentation and discussion of the Community Health Status Assessment,
- a live broadcast discussion with Richard Hofrichter, Ph.D., Senior Director for Health Equity at the National Association of County and City Health Officials (NACCHO) who among many publications, spearheaded the development of the online course ‘Roots of Health Inequity’ and
- review/ prioritization of health issues leading to a community health strategic plan for the next five years.
A WePLAN page is now available on the Cook County Department of Public Health website. All assessment documents including agendas, presentations, draft reports, summaries, etc. are posted and organized by meeting dates. We encourage you to check the website frequently for new documents and to review the previous findings to refresh your understanding and prepare you for the prioritization exercise at the next meeting.
