Chronic Disease Self Mangement Program: Curves, Blue Island

Are you an adult with an on-going chronic health condition like diabetes, hypertension, chronic pain, heart disease, anxiety, obesity or asthma?  Or, are you caring for someone with any of these health conditions? If so, the Take Charge of Your Health: Live Well, Be Well Workshop can help you.Take Charge of Your Health – Live Well, Be Well is a free 2 ½-hour six-week workshop. The classes are offered by the Cook County Department of Public Health and are led by Stanford University leaders with health conditions themselves.

 Participants will learn how to:

  • Manage chronic conditions
  • Practice good nutrition
  • Relax and handle difficult emotions
  • Problem solve around ongoing health
  • conditions
  • Set your own goal and make a step-by
  • step plan to improve your health
