About the Cook County CPPW Initiative

The Cook County initiative is a project of the Suburban Cook County Chronic Disease Prevention Network, a collaborative led by CCDPH. Its overall goals are to:

  • Increase availability, accessibility, and demand for nutritional options.
  • Decrease opportunities for excess caloric intake from unhealthy food and beverages.
  • Increase availability or utilization of safe places to be physically active.

Funding will be used to promote changes in policy and to the environment in school, workplace and community settings that make it easier for individuals and families to make healthy choices and live active lives. Click here for a preset menu of evidence-based or best practice strategies supported by the grant award.

We will work with:

  • Partners that represent various sectors vital to a community’s design and its health, including government, urban planning, parks and recreation, agriculture, health care, schools, businesses, grass-roots community- and faith-based organizations.
  • Health Campaigns to raise awareness about issues impacting nutrition and physical activity.
  • Trainings for communities to effectively advocate for local, county and state policies that result in environments where people can eat healthy foods and be physically active.
  • Grants to communities and schools interested in laying the groundwork to become model places to live, work and play. This initiative will be an expansion of the Model Communities program that is currently supported by funds from the Action Communities for Health, Innovation, and EnVironmental changE ACHIEVE initiative of the National Association of County and City Health Officials (NACCHO) (sponsored by the Centers for Disease Control and Prevention).
