The Cook County Department of Public Health investigates foodborne illnesses that were a result of eating in a food establishment in suburban Cook County (excluding Chicago, Oak Park, Stickney Township, Evanston and Skokie). If you feel you became ill as a result of eating at a food establishment in suburban Cook County, please fill out the form below and our staff will contact you. If outside of suburban Cook County, please contact that appropriate public health department.
