CCDPH requests your feedback on 2020 community health findings and priorities for suburban Cook County

The Cook County Department of Public Health is requesting your feedback on the WePLAN2020 Community Health Assessment (CHA) findings and health priorities. Following an extensive 9-month process and the involvement of over 2,200 residents and service providers, the assessment findings below (in English and Spanish) include:

Your answers are confidential and we do not ask for your name. The health department and the WePLAN2020 committees value your feedback and will take into consideration your opinions as they plan for health improvement. This is important because the report will be used to guide what the health department and community partners together will do to improve the health of all residents by reducing and eliminating unfair and preventable differences in suburban Cook County communities.
